Appointments
You may schedule an appointment with us by calling 215-762-5037 (press option 1). When calling please provide the following information:
The physician you wish to be scheduled with, if known
The purpose of your visit: new patient, routine follow up, sick visit, or annual physical exam
Your personal information (address, phone, emergency contact, etc
Your insurance coverage, including ID number and cardholder information
On the day of your visit please bring the following information:
Your insurance card and cardholder information
Photo Identification
Payment for your copay
Your current medication list
A list of questions you may wish to ask your physician
If you are a new patient you can download our new patient forms so that you can have them completed prior to your visit.
Insurance
Our office accepts most all insurance plans including Personal Choice, Blue Cross Blue Shield, Keystone Health Plan East, United Healthcare, and Aetna US Healthcare. To check if your insurance is accepted, please call our main number at 215-762-5037.
Billing
Your questions about billing are important to us. If you have any questions please call our billing office at 215-246-5437. The billing office is open Monday through Friday from 8:30 a.m. to 4:00 p.m.
