Form A Initial Advisory Committee – An initial advisory committee is formed for each student who pursues the Ph.D. degree within the first eight weeks of the student’s first registration in the Biomedical Graduate Program.

Form B Report of Committee Meetings – should be completed every 6 months at the Committee Meetings. This form should give the student comments, guidance and direction. The student and Chairperson of the Dissertation Examining Committee should sign the form and return it to the Office of Biomedical Graduate Studies immediately after every meeting.

Form C Preliminary Examination – This should be taken at the end of the first academic year. Students are required to pass the preliminary examination. The conduct and judgment of the performance in this examination is determined by the pertinent program’s discretion of an appropriately constituted examination committee. The prelim exam should be completed by July 1st of the first academic year.

Form D Qualifying Examination – to fulfill the requirements for candidacy for the Ph.D. degree. A student must pass the qualifying examination. This exam should be taken after completion of two academic years of graduate studies and at least 24 calendar weeks (six months) prior to presentation for the final thesis defense.

Form E Dissertation Committee – This committee should be formed by the end of the second academic year and will oversee the student’s dissertation research.

Form F Dissertation Proposal – After presentation of the dissertation proposal, this form should be completed and signed by all members of the Dissertation Committee and sent to the Office of Biomedical Graduate Studies.

Form G Notification of Intent to Defend – collects the information needed to create a notice announcing your dissertation defense. Please fill out this form and give it to your Program Coordinator. You must defend your dissertation on the Queen Lane or Center City Campus only.

Form H Dissertation Defense – After a successful defense of the dissertation, this form should be signed by all members of the Dissertation Committee and returned to the Office of Biomedical Graduate Studies.

Form I Certification of Dissertation – should be signed by your Dissertation Examining Committee Chairperson to certify that the dissertation you are submitting is satisfactory for publication. It should be brought to the Office of Biomedical Graduate Studies with the required four copies along with any personal copies of your dissertation to be bound. (Please see current academic calendar for deadlines).

Form J Commencement Information and Forwarding Address – should be completed and returned to the Office of Biomedical Graduate Studies before you leave Drexel University College of Medicine. Please complete all the information that is requested whether you plan to attend Commencement or not.
