Patients who receive primary medical care for HIV/AIDS from the Partnership Comprehensive Care Practice (Partnership) are eligible to receive Case Management Services. Case Managers at the Partnership are available to help patients get what they need to live with HIV infection or AIDS. An HIV/AIDS Case Manager will keep patient's HIV status private and only disclose HIV-related information, with a patient's written permission, when necessary to access requested services.
The Case Manager, in partnership with the patient, will assist in creating support systems and resources to fulfill unmet needs. Case Managers are available to travel to the patient's home, to the hospital or to another agency or community center if needed. Appointments can be made with a Case Manager in his/her office in the clinic.
A Case Manager will work closely with the patient to assist in accessing information about community resources that are available to help meet needs. These resources address a number of areas including medical, economic, and basic needs, such as:

Specialized Case Management Services
Case Manager Assistant – Assist patients with accessing medical insurance, Social Security Benefits, Special Pharmaceutical Benefits Program. The CMA is also available to accompany patients to appointments to offer guidance and support.
Inpatient Case Manager – Collaborates with the Partnership medical providers and care team to arrange discharge planning for Partnership patients hospitalized at Hahnemann University Hospital.
Prenatal Program – In collaboration with the Women's Care Center at Hahnemann University Hospital, a Case Manager will work with pregnant women to reduce the transmission of HIV to the infant. A Case Manager will offer support and assistance in accessing resources for pregnancy.
A patient can only have one HIV Case Manager. If a patient decides that he/she needs Case Management Services, a patient can request to speak with a Case Manager during their scheduled medical appointment at the Partnership or contact the Director of Case Management, Sonia Massey, MSW. In addition, the Uniform Access Project has been created by the City of Philadelphia's "AIDS Activities Coordinating Office" to help link people with HIV Case Management. Therefore, the Uniform Access Project (1-800-929-5602) must also be contacted and notified by the patient that the Partnership is providing their primary medical care and they would like to receive Case Management Services. The patient will then be linked to a Case Manager at the Partnership or placed on a waiting list, if necessary.
